Successful applicants will be issued with a “Conditional Offer”. To fulfil the Condition of Admission, they must submit the original “School Head’s Recommendations” by mail and attend an Orientation Session. Otherwise, their conditional offers will become null and void.
獲取錄者將獲「暫取生」資格。暫取生必須寄交「校長推薦書」正本，並且出席新生日。否則，其「暫取」資格將會自動失效。

1. I declare that all information given in support of this application is accurate and complete, and understand that any misrepresentation will result in disqualification of my application for admission and subsequent enrolment in the University. 本人謹聲明在本申請表中填報之資料均正確無誤，亦無遺漏，並明白若填報之資料失實，本人之入學申請及修業資格將被取消。

2. I consent that if registered, I will conform to the Statutes and Regulations of the University in force. 本人同意一經註冊，本人將遵循大學的規章和法則。

3. I understand that, upon my registration in a programme, the data will become a part of my student record and may be used for all purposes relating to this course in accordance with the procedures of the University. 本人明白在註冊後，有關資料將轉為學生紀錄，而香港中文大學可按校方規定，將該等資料用於與本課程相關之一切事宜上。

4. I understand that applicants who have previously attended this course or related courses will not be considered. 本人明白曾修讀本課程或相關證書／學位課程的申請人，將不獲接納。

5. I understand that my application is accepted and based on the recommendation of my current (2024/25 school year) school head. If I change my serving school in the 2025/26 school year, my admission to the course may be cancelled. 本人明白本課程取錄的條件之一為申請人必須獲現職（2024/25學年）學校校長推薦，如本人於2025/26學年轉教他校，本人的入學資格或將會被取消。
